You can contact the church where you were baptised or confirmed and the Priest should have all of those documents on record.

If you know what city you were baptised in, you should be able to contact that particular Diocese, and they should be able to look through their records and find it. Each Diocese keeps records of every baptism they perform during each year. So even if you do not know the specific church, the diocese should find it.
