Why is my Ford Ranger's engine running rough at idle?

The problem might be due to an open lifter that failures to bleed down or flow properly. Therefore, you should check the engine oil level. In order to run smoothly, engines with hydraulic lifter must have clean flowing oil. Therefore, you should follow the recommended scheduled plan for oil changes as well as use proper engine oil weight.

Most likely needs a good tune up. Change plugs, wires, fuel filter. Service or clean air filter and check all fuel and vacuum hoses for leaks, cracks or broken fittings. Spray WD-40 or carb cleaner around plenum and make sure rpm's don't change. If they do you may have a faulty plenum gasket which will require changing.

could need a tune up,or you may need to clean your fuel delivery system with a good fuel treatment product. but most of the time this rough idle is cured with both methods being used in conjunction with each other.

Your throttle positioning sensor sounds bad. You need to add throttle body cleaner. Also try adding some fuel injector cleaner.
