When someone asks if you're in "full form", what do they mean?

What is the origin of this phrase?

Being in "full form" means that you feel completely well; healthy both mentally and physically. Some other ways of phrasing this are: feeling great, feeling 100%, feeling well, being healthy.

To be in "full form" means that you are at top capacity; that you are performing absolutely as well as you are able to, in all regards.
