It depends on where your molar is. If it's under the skin, it may require cutting the skin open. If it's under the bone, it might require heavy surgery. In the easiest cases, it's a matter of simply popping the tooth out and staunching the blood flow, then being careful until it heals.

There is anesthesia either local or general depending on the extend of the extraction. The tooth is often broken in pieces then removed all the way down to the root.

Your dentist first puts a topical on your gums near the area. Then he injects a numbing agent into the area in your gums near the tooth. Finally he basically gets his tools around the tooth to loosen it up. He will yank on your tooth and pull out both the tooth and roots.

A molar extraction can be simple or complex depending on the tooth involved. If the tooth is easily accessible, then the tooth is extracted using specialized forceps and steady pressure to remove it. If the tooth is difficult to reach or has broken below the gum line, then a more surgical approach is used to extract the molar.

With a simple extraction, local anesthetics are used and they use simple instruments to take out the tooth. With a surgical extraction they have to do a full incision because the tooth is either invisible or can't easily be reached in a dentist's chair.
