Well you can get an annulment if you'd like, or if you want to change it back just go to the court house and apply for change of name, also they usually ask why so you must have a good reason.

You should just be able to go to your local courthouse and fill out some paperwork. I would suggest bringing your birth certificae or identification just in case it's needed.

To change back to your maiden name you just have to go back to a government office with your divorce decree and something with your maiden name, like your birth certificate and change it back.

File a petition for change of name at your local court. Contact local newspaper to have them publish the name change. Take proof of this announcement to the court and then it will be official. Last get your name changed on important documents (Drivers license, Social Security card, etc).
