What is the history of modern medicine?

I am trying to get a handle on the history of modern medicine and how it got started, if it is rooted in witch craft.

Modern medicine began with a rejection of herbalism, the "four humours," and mindlessly following traditional figures of authority and an emphasis on antibiotics and the transmission of disease by bacteria.

There are loans of websites and books regarding the origins of modern medicine. Generally, Hippocrates and Avicenna, a great thinker and scholar of medicine and author of "The Canon of Medicine" written in 1020, are considered the fathers of modern medicine. Illness was thought to be a function of witchcraft.
