What is involved in getting someone with Parkinson into a hospice?

I need hospice information.

To qualify for a hospice placement, the person must be expected to live for less than six months. Please contact your local hospice for detailed criteria in your area.

Hospice care is available for individuals not expected to live longer than six months. Affliction or diagnosis should be irrelivent and a family doctor can provide a hospice agency referral.

Hospice is end of life care for a patient that is expected to live less than 6 months. Only a doctor can make this assesment. If you doctor has made that assesment he/she will be able to direct you to a hospice. If not, you probably mean you want nursing home or in home health care providers.
