Bilirubin is not a causative agent in liver cancer. Many types of cancer in the liver do not originate in the liver but in other organs and spread (metastasize) to the liver. Both these secondary and primary types of cancer can cause elevation of serum bilirubin when they obstruct the biliary system.

The body should normally contain levels of bilirubin ranging from 2 - 20 mol/L. A high level of bilirubin can be an indication of liver cancer. If bilirubin builds up in the body, the liver cannot function normally and will develop hepatitis or jaundice. If jaundice develops then there is almost a certainty of liver disease.

Liver cancer causes damage to the tissues inside the liver, limiting the effectiveness of the liver's ability to remove bilirubin from the bloodstream.
