What is a service certificate?

According to the law what constitutes an official service certificate?

A service certificate goes at the end of a pleading, after the signature block. It certifies the recipients of services copies of the document. The certificate must contain the date sent, the method sent (US mail, fax, e-mail, personal delivery, etc.), and the name of the recipient along with the applicable address, fax number and/or e-mail address.

It's a notarized statement of the events of the service: where the service occurred, how the person was served, and the date/time of service.
