What documents do I need to take with me to a plasma donation center?

I have never donated plasma before.

You'll need proof of identity and, if you've had the Hepatitis B shot, proof of that, as you'll be paid more if you've had the vaccine. You will undergo a complete physical and give a detailed health history. The first visit will take 2-3 hours. Eat a full meal 1-2 hours before you go, and take along a snack.

A birth certificate and photo id is all you need to register there and they will give you a card that will identify you to the center for subsequent visits.

Proof of address which includes proof of residency in the county, such as a phone bill or rental lease. Proof of identification, which means Driver's license or state ID card. If you're a student, bring your student photo ID as well. Also will need birth certificate, possible documentation from a doctor stating all medical history and proof of health insurance.

Basic guidelines: must be between 18-65 years old, must weigh at least 110 lbs and generally good health. Documentation not standardized for every state. Call the center and ask what documents you need to bring. Some require only a pictured ID. Others want SS card and proof of residence.
