On a 30 day move out notice, include your name and anybody else on the lease, your address, the current date, the date of move out, and intent to move.

It depends on what you are writing a 30-day notice for. If it is to a landlord, for example: "Please accept this letter as the 30-day notice of my plans to vacate the property on [date]. I will contact you to arrange a walk-thru of the property. Thank you." Keep a copy.

Forms are available but not necessary. Include the sentence "I'm giving 30 days notice that I will be moving out of (address). I will be out of the (apt/house/etc) by (date). My forwarding address is (new address). Thank You, Your signature. Mail/deliver a copy to your landlord. Make arrangements with the landlord about showing your place to new tenants.
