What could be going on if my four year old has a very high white blood count?

We recently had extensive blood work done on my four year old as a result of him being sick for an extended period of time, the only thing that came back was a high white blood count.

A very high white count in your four year old child is virtually always due to an infection, which may be viral, bacterial, fungal or other parasite, though a viral etiology is certainly the most common. If there is however no infection, it is most certainly due to dehydration in a young child.

High white count can mean anything from your child having an infection to possibility of leukemia. You should discuss this with your child's doctor as soon as possible.

Assuming there isn't leukemia, a high WBC in and of itself just indicates there is an infectious process at work. ANY infectious disease would elevate the WBC.
