What causes human hermaphrodites?

There are 3 types of hermaphrodites: True, Male pseudo, and Female pseudo. True has no known cause as of yet. Male pseudo is caused by Androgen Insensitivity Syndrome (the body doesn't respond to androgen being produced). female is caused by Congenital Adrenal Hyperplasia (adrenal glands produce too much testosterone).

A hermaphrodite is essentially a human being that possesses the sexual reproduction system of both a male and a female. I believe this is due to an embryo being subjected, in the womb, to developmental hormone profiles for both males and females. Generally hermaphrodites present visually as females since the male sex organs develop internally instead of externally.
