Thermonuclear war could happen any number of different ways, but the most likely is an exchange of nuclear weapons between two atomic armed countries. A full scale thermonuclear war would kille hundreds of millions or billions of people. A single hydrogen bomb can destroy an entire city.

It's normally jsut known as "Nuclear war". It would happen when two or more countries with nuclear weapons came to the point where they were willing and able to use them, then did. It could happen any way wars normally happen. It's technically not a thermonuclear war if only one country uses nuclear weapons.
