First of all never use a microwave to heat a bottle, it create dangerous hot pockets and uneven heating. the best way is to run the bottle underhot tap water or place the bottle into a bowl of warm/hot water. Always shake and sprinkle on the underside of your wrist to double check temperature before serving to baby.

This would completely depend on what the baby is used to receiving on a regular basis. If the baby is breastfed, then the idea is for the bottle to be body temperature. This should be tested on the inside of your wrist and should feel like nothing to warm. Otherwise, the bottle can be cold depending on her preference.
