Facial bone contouring during plastic surgery is usually accomplished by the surgeon making an incision under the chin or in the mouth to reach the bones of the face. Then the bones are carefully repositioned or shaved down in order to reach the contours or dimensions necessary to improve the balance or symmetry of the face.

As grotesque as it sounds they basically use the same tools a sculpture would use. Mainly the old fashion chisle and hammer. The surgeon exposes the bone via disection planes and then goes to work eye balling it from side to side. They can also implant bone from other areas if they wish to fill and not take away.

It obviously depends upon the underlying problem but if a bone has to remodeled then it has to be broken first. This is done with special medical saws or drills. The cut bones (after remodeling and removing excessive bone) are then screwed in place with a variety of metal plates and screws.

As grewsome as it sounds they do it with chisels and picks. They basically use wood working instruments that are surgical grade. It is a good thing you are out and can't hear or feel.
