How do they come up with organizational structures?

Don't they add and take away positions all the time?

The management of any business constantly evaluates the profitability of the business, customer needs and any issues/or problems the business is facing. The structure of the organization is fine-tuned periodically to meet the needs of the customers, the employees and the goals of the business.

Organizational structure depends entirely on the industry. Some industries operate better using a vertical structure (such as small family owned businesses), while others work better using a matrix structure (more complex, multi-subsidiary organizations).
