Actually a minor can not change their name unless they go through a court system, and a judge has to approve it.

We've actually had a daughter do this when she was 16. She went and got the paperwork to be filled out for name change, took it to the courthouse and paid the filling fee's, this was either accepted or rejected, in her case it was accepted. And she had a new legal last name.
