It depends on what type of hoof disease it is. Thrush is common- treat with thorough cleaning, scrubbing with warm water, and the application of povodine iodine scrubbed in. White Line Disease is treated with White Lightning. Too many diseases to list.

Various hoof diseases require different treatments, but all require a healthy diet/supplements, and regular farrier visits. There are brush-on medications available for such things as Thrush. Brittle hoof can be helped by keeping the ground wet near the water trough. Hoof rot requires a clean/dry area.

It depends on the disease. You can use a product like Easyboots to ease concussion pain and apply a cooling poultice to help draw out heat from the hoof. If it is navicular, adding pads under the shoes to help place the weight forward off of the navicular bone helps.

You are going to need to consult your Vet and Farrier about how to approach this issue. This horse is probably going to need frequent hoof trimming and probably put on a good hoof supplement.
