You'll need to contact a reliable building moving company in your area, they will coordinate with you to get it done. They'll put steel beams under the house, lift it off the foundation before sliding it onto wheels and then attach it to a semi for transport.

I have actually had this done. Hire a house moving company and the will secure the house and lift it a moved to any location, preferably close because it gets expensive.

It is possible to move a house that is over a crawlspace or basement. Several steel beams may have to be added under the main floor. Sometimes, cables are attached to those beams so the house can be lifted onto a wide truck bed for transport.

Contract a house mover. He will raise the house and put it on beams and lift the house, moving it onto a flat bed truck especially made to move houses. There are many permits to get for this project and the city needs to get involved.

House moving can be a very complex task with a high degree of risk causing serious(expensive) house damage. Unless the mover is very competent this is a job that should be done by professionals well versed in the process.
