How are television subtitles produced?

They seem to contain many errors and misspellings. Background music is often wrongly-titled.

Some television titles are human typed as they air, which can lead to errors. Also, some computer programs translate the audio into text, which can also lead to errors. Most of the time a prerecorded TV show will be done correctly, but live TV (such as the news) can have errors or words not in the script.

Television subtitles are produced by subtitling companies. For live television they have to do is virtual as they see it, so there will be errors.

I suspect they are partly automatic and partly monitored by humans otherwise I don't know.
