Can a cat urinating *by* the litterbox be a sign of kidney disease?

Yes, there can be kidney stones or bladder stones or it can be behavioral. It is important to first have your veterinarian rule out physical causes, then go from there.

My kitty had kidney issues for 10 years and never did that. This could be a sign that your cat doesn't like either the brand or type of cat litter being used or that the current kitty litter isn't clean enough. If using perfumed kitty litter try some that isn't.

Urinating by the litter box instead of in it is not a sign of kidney disease. Cats will usually do this if the litter box needs to be emptied or if they are unhappy for some reason. The first sign of kidney disease in a cat is increased thirst.

Yes. Any urinating out of the norm should be taken note of. It can be the result of pain. Go to the vet.
