Mainly because IVF is a voluntary procedure, falling into the same sorts of categories as plastic surgery, weight loss surgery, etc. , insurance companies are typically not required to cover it. Several companies do, at a price. Depending on your state, there might be other terms.

Laws vary from state to state. Very few, if any, states mandate coverage for any fetility treatments, however, including IVF. You should double-check your state's laws, however.

This varies from state to state. The State Board of Insurance may be able to help you find out more information. Most states leave this up to the insurance companies and their agreements with individual employers.
